Refer your Patients to a Registered Dietitian

• Patient demographics: Name, date of birth, address, phone number and email address
• Insurance information: Insurance plan and member ID
• Reason for referral: Include the applicable ICD-10 diagnosis code(s)
• Referring provider: Provide name and contact information
• Relevant clinical documentation: Medical history and other pertinent clinical notes
• Current medications: Medication list relevant to the patient's condition
• Laboratory results: Recent labs relevant to the patient's condition

Using Athena?

If the referring provider participates with Athena, we are able to access the patient's prior medical documentation directly through Athena. Additional records may not need to be faxed if they are avaialble to us through Athena.
